To order embroidery for your scrub items, fill in the following order items. You will need to complete this page for each individual item you wished embroidered.

You will be asked to choose the text you want (up to 3 lines per side), the thread color, and the type face.

Be sure to indicate upper and lower case letters as you want them on your embroidery. Please note that script cannot be all uppercase.
